Submitting a Story for the J/C Index

Note: Everything listed in red is required information, everything in green is optional. However, I would ask you to fill in as much of the information as you can, since it would make the index more complete and would also give everyone a better understanding of what they'll be getting themselves into by downloading or requesting your story.
